の有用性について検討した。【対象】2004年12月~2006年8月の間に手術が施行された非小細胞肺癌58症例(腺癌37症例、扁平上皮癌18症例、他3症例)(N2 14症例、N1 15症例、N0 29症例)を対象とし、病理にてリンパ節転移の有無が診断された計348領域のリンパ節(縦隔 180領域、肺門部 168領域)について検討を行った。【方法】対象のうち、46領域(縦隔 17領域、肺門部 29領域)で転移が確認された。PET-CTによる撮像をFDG投与後1時間(早期相)で行い、胸部に関しては投与後2時間(後期相)で再度撮像した。各リンパ節領域のSUV maxを早期相、後期相でそれぞれ計測し、視覚的に集積が認識できるリンパ節領域(今回の検討では早期 SUV max が1.5以上のものを対象とした)に関しては、早期相から後期相へのSUV max の増加率を算出した。さらにSUV max と増加率を用いて、リンパ節転移の検出能を検討した。【結果】SUV max の増加率の平均は、縦隔において転移ありのリンパ節領域 18.69%、転移なしのリンパ節領域 -0.49%。肺門部において転移ありのリンパ節領域 15.95%、転移なしのリンパ節領域 1.62%であった。リンパ節転移の有無でSUV max の増加率にそれぞれ有意差を認めた。SUV max 2.5、2.0をカットオフ値として、感度、特異度、正診率を算出すると、それぞれ32.6%, 94.7%, 86.5%、50.0%, 82.8%, 78.4%であった。これに増加率>10%を組み合わせると、それぞれ31.0%, 98.0%, 88.8%、47.8%, 95.7%,
