The refinement of the surgical technique and the discovery of more effective immunosuppressive agent have led to an improvement in results of liver transplantation, and now it has become the accepted mode of therapy for the patients with end - stage liver diseases. But it is the fact that even now not a few problems are remaining in some aspects of liver transplantation. In this article, we discuss several problems regarding patient selection, organ procurement, and surgical complication, such as the appropriate time of operation in cirrhotic patients, indication for malignancy, segmental transplantation, thrombosis of the hepatic artery, on the basis of the recent reports from Pittsburg, Cambridge, and Hannover.
