A 67-year-old male patient with diabetes mellitus and liver cirrhosis complained of itching of the skin, and boric acid was prescribed to be used for topical cleansing in 2% solution. However, it was given in an envelope for oral use together with the other internal medicine, he ingested l0g of boric acid 5 times during a period of 2 days. Meantime the patient developed retching, vomiting, abdominal pain and diarrhea and was admitted to the hospital. After an additional ingestion of l0g the symptoms became increasingly severe and erythematous eruptions appeared on the face and forearms. Next day the patient exhibited convulsions, delirium and coma and on the third hospital day died from circulatory collapse and renal failure. The deceased was buried, but exhumated for medicolegal autopsy 8 days after death. The body' was so decomposed that it was difficult to observe the pathologic findings. Nevertheless, boric acid was demonstrated from the cardiac blood, urine and gastric content by the curcumin method. The cause of death was determined to be acute boric acid poisoning resulting from oral ingestion by therapeutic accident. The present case warns of the potential danger of boric acid, especially when orally ingested. |
