| 内容記述 | Following water-soluble ionic iodinated contrast media, amidotrizoic acid (AMI), iotalamic acid (IOT) and iodamide (IOD), were analyzed using thin layer chromatography (TLC), high performance liquid chromatography (HPLC), gas chromatography (GC) and gas chromatography/mass spectrometry (GC/MS). These iodinated contrast media were qualitatively identified by TLC, HPLC, GC and GC/MS, but simultaneously quantitated only by HPLC. On gas chromatogram, IOT and IOD produced two peaks of the trimethylsilyl derivatives, whereas AMI gave a single peak. Therefore, quantitation by GC was successful for AMI, but not for IOT and IOD. After myelography a 50-year-old female patient progressively developed clonic convulsions, dyspnea and vomiting and a few hours later died. From the cerebrospinal fluid of the deceased taken by cisternal puncture at medicolegal autopsy, AMI was detected at 13.45 mg/ml by HPLC and at 12.99 mg/ml by GC and GC/MS. The deceased was thus demonstrated to have received inadvertently an intrathecal injection of Urografin (AMI) instead of non-ionic Isovist(Iotrolan) during myelography and to have died of cardio-respiratory failure due to hyperosmotic effects on the nervous system. | |||||||||||
